2/22/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning observations, the client sat quietly in his seat during the class math discussion and raised his hand before speaking. During midday observations, the client exhibited difficulty complying with directives on the first prompt by refusing to complete his class work. During afternoon observations, the client interacted with his peers in a positive manner while outside for recess. QMHP-C greeted the client upon arrival to the school setting. During morning observations, QMHP-C provided the client with positive reinforcement and encouraged him to continue sitting quietly in his seat and to raise his hand before speaking in class. During midday observations, QMHP-C redirected the client and reiterated the importance of complying with directives on the first prompt and completing his class work. During afternoon observations, QMHP-C provided the client with verbal praise and encouraged him to continue interacting with his peers in a positive manner. The client actively engaged in Therapeutic Day Treatment services throughout the day. During morning observations, the client agreed to continue to sit quietly in his seat during the class math discussion and to raise his hand before speaking. During midday observations, the client was receptive to redirection and agreed to comply with directives on the first prompt. the client was then able to calm down and complete all of his assignments. During afternoon observations, the client agreed to continue interacting with his peers in a positive manner while outside for recess. The client also agreed to continue to track his behavior on his self-monitoring chart.
